In order to be eligible to participate in athletics, 7th and 8th grade students must submit the following, prior to the season starting. This does not need to be repeated each season. If a student athlete participates in multiple seasons, the items listed below must be submitted prior to beginning the first season in which they participate.

1. OPS Middle Schools Middle Level Activities Contract (below, right document)
2. Student and Parent Consent Form (below, left document- within the packet- English and Spanish versions available)
3. Sports Physical completed AFTER May 1st 2024
   1. The OPS provided form can be submitted (below, left document- within the packet- English and Spanish versions available) or documentation from a licensed physican stating that the student is cleared to particapte in school compettive sports programs can be submitted
4. Concussion Fact Sheet for Students and Parents (below, left document- within the packet- English and Spanish versions available)
5. Insurance Coverage Form (below, left document- within the packet- English and Spanish versions available)
