    A physical exam is required by Nebraska law for all students entering Pre-K, kindergarten, seventh grade and/or transferring into Omaha Public Schools from out-of-state. Required school physical exams are to be done on or after March 1 of the upcoming school year.


    Students who wish to participate in athletics will also be required to complete a sports physical. Below are the guidelines for each grade level where a physical is needed.  


    School Physical for Pre-K, Kindergarten or transfer from out of state:
    A physical exam and vision exam is required for all students entering Pre-K or kindergarten and/or transferring from outside the state. Exams can be completed up to six months prior to entrance into schools or upon transfer into Omaha Public Schools from outside the state. The vision exam may be completed by a health care provider or an optometrist. A complete dental check-up is strongly encouraged at this time also. Required school physical exams are to be done on or after March 1 of the upcoming school year. Your child's physician will give you the documentation required for a school physical. 


    School Physical for Seventh Grade:
    A physical exam is required for all students entering into seventh grade without the vision exam requirement. Completing the seventh grade physical also meets the requirement for athletic participation in after-school sports. Your child's physician will give you the documentation required for a school physical. 


    Physicals for Athletic Participation (Eighth-12th Grades)
    Students who wish to participate in after-school sports for eighth through 12th grade are also required to have a sports physical. The physical can be done on or after May 1 of the upcoming school year. For more information, please contact the coach or Athletic Director.


    Exemption Waiver
    If you do not wish to provide evidence of a physical exam and/or a vision exam, a signed statement waiving this requirement must be submitted to the school by Nov. 1 (or 60 days after enrollment). Please note, if your child will be participating in competitive athletics, a physical examination by a licensed health provider is required and signing a waiver does not satisfy this requirement.
