iPad Consent Form and Insurance

Make sure to complete the green iPad consent form.  If your student is new to OPS, this completed form is needed before your student can be issued an iPad.  If you would like to purchase iPad device protection for the 2023-2024 school year, make sure to send $20.00.  The iPad insurance money is optional, but the green consent form is required.
