*Birth Certificate - Your student must turn 5 on or before July 31, 2023

*Proof of Address - Bill from OPPD, MUD, or Cox, Rental or Purchase Agreement for house

*Immunization Record

DUAL LANGUAGE:

If you are interested in the Dual Language Program, please fill out the Dual Language Application at this time. The lottery will take place in May.

*If your child has an older sibling in the Dual Language program, you must complete an application with siblings noted on the back.
