Before finishing and submitting the application, the applicant will be asked to either select:

I, the applicant, confirm that all of my answers to the questions in this employment application are accurate and complete.I understand that employment consideration will be contingent upon the accuracy, completeness, and acceptability of the information furnished, and the falsification of this application in any detail is grounds for disqualification from further consideration or for dismissal from employment.

I authorize any former employer, educational institution, government unit, Nebraska Child Abuse/Neglect Central Registry or other person or entity having such records or information concerning me to furnish such records or information requested by the Omaha Public Schools or their duly authorized representative.I understand in executing this authorization I waive the right of such person or entity from all liability in connection with such release of information.

All statements contained in this application are considered a part of the working agreement if the applicant is selected. OPS Background Check Process

During the application process applicants will be asked to provide the following information for the purposes of the OPS background check process only:

  • Maiden Name (Or other names used)
  • Social Security Number / National ID
  • Driver's License Information - Required for Transportation Applicants Only
  • Previous U.S. Addresses (up to 20 years)
  • Names & Birthdates of Children who have lived with you
  • Applicant Date of Birth
