Dear Staff:
This is just a note to welcome you to the 2009-10 School year and to let you know how the Risk/Safety Management Department can help you. Feel free to call us if you need our assistance any time during the year.
The Risk Management Department is here to help you in the following ways:
1. Workers Compensation Insurance – If you or one of your employees are physically hurt while performing job duties, either on or off school premises, the injured person (if able) needs to fill out an “Employee Job Related Accident Report” and send it to the Risk Management Department, TAC through the school mail.

You can download the Employee Job Related Accident Form from the Workers Compensation link. Additional copies may be photocopied or ordered from Supply, Item no. 1632.

If the employee needs to seek medical treatment, we have a managed care plan and the employee needs to notify our nurse case manager, 1-888-354-1015. The employee may go to his/her own doctor only if the doctor agrees to the managed care plan rules (the nurse case manager will handle this for them when they call her), or if they need immediate non-emergency treatment, we can get them into a doctor within the plan very quickly or to Methodist Emergency Room.
2. Student Injury Reports – Student injury reports are filled out and the original is sent to the Risk/Safety Management office. The copy is retained at school in the nurses’ office. The person with the most knowledge of how the student injury occurred should fill out the report. Fill out an injury report if the student needs medical treatment (from a doctor) or misses school due to the injury sustained at school.

If a parent or guardian wants a copy of the student injury report they may call our office for a copy.

If a parent or guardian wants to file a claim for payment of medical bills, etc. for a student injury, have them call our office. We will fill out the necessary paperwork for the insurance company for them. Our insurance company only pays for injuries that were caused by Omaha Public Schools in some way. So be aware that normal everyday childhood injuries will not be covered for the parents, but we will file their claim anyway if they wish.

If the injury is serious in nature, the staff and students who witnessed what happened should fill out witness reports. Please send copies of these in to our office also. (These can be on a plain piece of paper with the students or employees name printed then signed.
3. Visitor Injuries – We do not have a special form for visitor injuries, just use a student form. Cross out “Student’ at the top and write in “Visitor”. Send the reports to our office as you do the Student Injury forms. Call us if the injury is serious or an insurance claim should be filed.
4. Safety – We have a Fire Safety Specialist, Fred Clough and a Safety Specialist, Delicia Holland. They are available to help you with:

Building safety and fire safety inspections.

Traffic safety analysis around your building.

Employee and student safety. Playground safety and OPS playground equipment safety requirements.

For injuries that appear life threatening, do not hesitate to call 911 and have the employee transported to the nearest hospital.
