Therefore, a proposed notification letter must be attached to the research proposal. Written consent may also be required from active participants, parents/guardians of students under the age of 18, and district personnel, including teachers, who participate in the research when the research project involves identified students or access to student records, identified personnel or access to personnel records, or student surveys that include questions revealing information referenced in Section 11.05 – Policy and Regulations of the School District of Omaha and the Protection of Pupil Rights Amendment
